The Human Services and Public Health area is seeking a Senior Social Worker to join the Children's Mental Health Case Management team. In this position, you will be responsible for working closely with youth and their parent/guardians to address complex mental health needs while providing assessments, referrals, monitoring, and regular contact with the youth, families, and community providers.


The operated team of case managers work with youth who may be involved in multiple systems, such as Child Protection and Juvenile Justice. In this role you may interact with Child Protection, the Treatment Placement Screening team, Child Welfare, Corrections, and the Waiver system in addition to working with voluntary and involuntary parents/guardians who have children between the ages of 12-18 with mental health diagnoses (Post Traumatic Stress Disorder, Reactive Attachment Disorder, Major Depressive Disorder) and have experienced adverse childhood events.

Location and hours:

This position is hybrid and will include home visits within the community and remote work as job duties require. Work hours are Monday through Friday, 8 a.m. to 4:30 p.m. with room for flexibility. This position may have minimal out of state travel. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position you will:

  • Provide mental health targeted case management to youth with mental health diagnosis and engage them and their families with cultural humility and a person-centered approach.
  • Work with families to assess needs and strengths, resources and collaboratively develop short and long-term treatment goals and services plans.
  • Prepare and maintain case records, case finding, correspondence, and reports.
  • Work collaboratively with the interdisciplinary team and effectively with in-patient treatment teams, community providers, and families.
Need to have:

One of the following:

  • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
  • Bachelor's degree in social work, psychology, sociology, or human services, and 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
  • Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles or social welfare.
  • A valid driver's license and daily access to reliable transportation to travel primarily within/outside of the metro area to meet with clients in their residential settings and/or perform authorized off-site work duties.

    • Norms and values of varying cultural and socioeconomic backgrounds.
  • Ability to:
    • Communicate with, and understand the problems faced by families from all cultural background and economic levels.
    • Utilize supervision, consultation, and in-service training in achieving an increasing level of competence
    • Read, interpret, and apply policies, regulations, and procedures.
    • Effectively communicate verbally and in writing.
  • Skilled in using personal computers.
