**Responsibilities:**
As a **Care Manager II**, you'll empower members suitable for care coordination and case management services to achieve their best possible health. You'll engage members and providers to assess, plan, and establish individual goals. Here's a breakdown of your role:
- Serve as the single point of contact and advocate for members in our care coordination program.
- Conduct comprehensive assessments of environmental, psychosocial, and support needs.
- Identify problems/barriers and develop appropriate care management interventions.
- Create a plan of care with short and long-term goals, sharing these with members and families as needed.
- Implement interventions based on members' needs and clinical progress, scheduling follow-ups, making referrals, and documenting progress.
- Coordinate care and services with members, families, PCPs, specialists, and providers.
**Education & Experience:**
- Must reside within 30 minutes of Philadelphia County, Pennsylvania for in-person member meetings.
- Bachelor's Degree in Nursing or a healthcare-related field.
- Current, unrestricted Registered Nurse licensure in Pennsylvania.
- 3+ years of experience as a case manager preferred, with a minimum of 3 years of professional clinical practice as an RN. Preferred settings include Emergency Department or home care.
- Behavioral health experience, including managing care for individuals with mental health and substance use disorders, life stressors, crises, and stress-related physical symptoms.
- Proficiency in MS Office (Word, Excel, Outlook), internet applications, and electronic medical record and documentation programs.
- Eligible for Certification in Case Management within 12 months of accepting the position, with successful attainment required.
- Valid driver's license and car insurance.
