Family Caregiver Advocate

Job Function:

We are searching for a Family Caregiver Advocate to add to our team! The ideal candidate would be someone who would a career in public health and social work.

Responsibilities:

• Assesses and prioritizes caregivers for respite and/or supplemental services and evaluates requests for services to determine validity, appropriate service, and service priority

• Enters client information into the statewide data collection system(s)

• Provides impartial resource information and referral to family caregivers

• Maintains an adequate system for record keeping of individuals served, services provided and expenditures

• Identifies community resources and gaps in programs and services for caregivers and leads in developing resources and programs

• Continually evaluates the work plan for the regional Family Caregiver Support Program, including five identified services in Federal legislation.  

• Advocates for the interest of family caregivers

• Provides staff support to the regional advisory group to assist in evaluating the plan and creating active partnerships to enhance quality and availability of information, assistance and supportive services to caregivers

• Submits required reports to the state to document outcomes and expenditures

• Performs all other duties as assigned by the Executive Director

• Attends and participates in scheduled state office meetings, provider meetings, and staff meetings
